Thought this may be helpful to other people as well wanting to register as a minister/clergy or wedding officiant.
You just need to fax your letter of good standing and a copy of your certificate to the secretary of state.

You have to send a fax every year to be on file with the state. they don't send you a form or anything. it's just to notify the state only. You might include a cover letter letting them know your intentions. hope this helps
